Action commands will not bind (tested and working fine)

This problem keeps resurfacing: I can not get the following command to bind to ANY key button:

 

Sensor Select FWD: INS, IRMV/EOMV

 

See the attached screen shot and you'll notice the associated line is colored differently than all others.

 

The crazy part is, the problem came, went away and is now back - same exact action item, same problem.

 

Color me frustrated! Any help would be appreciated.
