Green dot and a big black square

I have not used the "Way" in a few months.  I downloaded and installed the latest version and have the Green dot on the F10 map, but the dot is inside of a large black square obscuring the exact target location.  Any idea's when is happening?

I've never seen this.

Remove all mods, run a repair and see if it helped.

Or is there a specific situation where it occurs? Can you consistently reproduce it?

It's consistently reproducible in every mission I have in my saved games folder.
