Can't get Next Level Racing V3 to work with A-10C

I've selected the DCS profile as the input, chosen the pitch and bank axes, and pointed it to the .exe file, but I can't get it working. DCS world works fine with it, so it is not the V3, just my rubbish setup

 

Anyone set this up?
