Multi-Factor Authentication not sending out one time code

I set up multifactor auth recently on the main site, but when I try to login it is asking for the one time code, but never sending it. Any thoughts on this?

Mine's asking for Google Authenticator codes which I'm supplying to the login but they're being rejected.
