Initial Launch screen 1/2 way off top of monitor

I am running 3 monitors.  Center: 3440x1440, left/right 2560x1440.

I created a monitor profile for my 3 MFDs and it works fine. 

1. what file controls where this screen is displayed (see attached pic). ?

2. Another problem is: game display does not cover all 3 monitors.  Even if I set the overall width higher.  There is a bit less than 1/4 of the right monitor is black (far right edge)..
