It looks like a combination of factors. 

 

1. AI flights are bad at getting into formation at low speeds with the forming aircraft going a bit crazy.

2. On direct take-off to landing waypoints the AI seem to have narrow rules for what is in formation before climbing/accelerating, then when they do climb to RTB they seem to do it as individuals and not as a flight. 

3. On direct take-off to landing waypoints the AI maintain a slow speed and ignore any setting to try and change that until the above occurs. 

4. Literally adding a single waypoint with a reasonable speed allows the AI to re-form logically. 

The right man in the wrong place makes all the difference in the world.
