I was able to figure out the issue and reproduce it reliably.  The issue is after turning the comm 1 volume knob up, you need to re-set the preset on the DED. 
Start mission During active pause, note the comm one is channel 5 and comm 1 volume is off Turn up comm 1 volume knob Press space to start mission When prompted, press space to conatct Texaco - nothing happens Press Comm 1, then 5 then enter to -re-set the UHF channel 5 Press space - trigger works Love the campaign, was sad to get stuck like this but I'm excitedly back at it. Just bought your other campaigns too.  Cheers

SOLVED 
 
It was breaking on landing, not takeoff. My bad habit was that I always dropped the gear in the break, and didnt wait until below 250 kts (I thought the gear would wait for a safe speed to lower automatically). 
 
I suspect ED changed the damage model and this bad habit of mine finally got me in the butt. I was focusing on overspeeding on the takeoff, I never thought about my landing.
