How close do you have to be to an airfield to get a response when you request 'inbound'. I thought all the 10 airfields showing on the screen are in range and can be contacted.

I think all the 10 airfields are the max number of airfields the list can show. Its basically the 10 closest airfields. Other then that I cannot help you with your question.

 

Edit: If I understand it correctly, the first airfield is the closest one, the 10th are the one most far away from you.

You can request inbound from everywhere though I'm not sure which aircraft is declared clear first by the ATC. It is either the closest one or the one who radioed inbound first. But that ain't nice if you're a hundred miles out, request inbound and have the runway blocked for planes that are way closer. You can switch to an airdome that's not on the list by using the respective function key for Next Side or something like that. On the other hand it's something not often practiced unless you're playing on a server with strict ATC rules and/or LotAC.

 

Procedure is requesting navigation assistance to actually get to the airdome and once you're there, go on a go-around while checking for other aircrafts on approach or pattern and then requesting inbound. When you got clearence, proceed to landing. The kneeboard should help you with ATC charts for every single airfield and their corridors/routes.

I tried 150, 100 and 50 miles out to contact tower and couldn't get a reply when I call for a heading or inbound.

It is usually 15nm from your target airfield that one should call in Inbound.

 

What about calling in a 'request heading' when you're lost?

Same problem there.
