When I press the autopilot reset button it usually disables one or more of the axes on my cyclic or tail rotor pedals.

I'll be flying and set the force trim (autopilot hat forward) and can fly with the force trim engaged. But when I want to reset the trim and depress the trim had accordingly it almost always disables one of the four cyclic directions - usually cyclic forward. The consequence is of course that the helicopter, trimmed for forward flight now rears up and does not respond when the cyclic is pushed forward to correct it. If I look down at the cyclic and what where it is moving it will move in the other directions but looks like it is locked from moving forward. The solution is to pull in the opposite direction (ie. backwards on the cyclic) and then when I push forwards it responds normally (although by this time I've gained 200 ft in altitude and lost 30kts airspeed). I've experienced the same with the rotor pedals and you can see that one of them has locked and will not move - you need to press the other to free it up.

Whether it is connected or not I also note the the Attitude hold mode (trim hat left) is useless in the hover - simply doesn't work like it does for Wags in the video. It used to work a few weeks ago but now seems to do nothing.

There are a couple things going on here...

If you perform a "trim reset" while in forward flight, you can expect the nose to come up abruptly.  You need to anticipate this and quickly move the controls to the previously trimmed location to counter this, or only use the trim reset function when the virtual controls are near their center already, i.e. when in a hover, and not in full forward flight.

I posted a video in this thread on how to use Joystick Gremlin to avoid this problem when using Trim Reset. New-comers to the Apache might find it useful.

I haven't heard of others experiencing the issue with the axis being ignored like you are describing.  If you are using Central Position Trim Mode, is it possible you have some forward "drift" in your stick, and it is never registering as having been centered in the y-axis after applying the trim?  You can try a slight deadzone to see if that does the trick.

Your issue with the AT hold not working in a hover may also be because if drift in your joystick.  The Attitude Hold is currently very "weak" compared to previous patches (like when Wags created that video), and it doesn't take much input from the controls to get the aircraft to override the hold and start to move out of position.

Floyd has explained quite good already, but it would be helpful if you could record a video/track file of what's going on and also screenshot your special options for Apache.
