We have the same problem with one of our maps running on a dedicated server. If the same map is run on a client (acting as a server) every thing is fine.

It appears to be something to do with numbers of players attached, we had about 6 people join before it went bang (so its a timing/scheduling thing). The map has no scripting in it unlike deadlyfish's.
