I was also experiencing microstutters while panning through the cockpit despite having stable 60 FPS and the solution for me was setting Maximum pre-rendered frames to 1 in Nvidia control panel with Vsync set to adaptive, in game Vsync also on and without hitting Lalt+enter. The only major fps drop happens near trees but all my shadow options are set to high.

I had exact same issue as you, followed your settings and the MiG-21 is out of the hangar and flyable again :D

VIRPIL VPC WarBRD Base with HOTAS Warthog Stick and Warthog Throttle, VIRPIL ACE Interceptor Pedals, VIRPIL VPC Rotor TCS Plus Base with a Hawk-60 Grip, HP Reverb G2.
