As the title says, where do I make a formal complaint about forum moderation?

Somewhere (was it in the forum rules?) was stated, in case of a dispute, contact the moderator in question per PM. If that does not help, contact one of the Community Managers.

 

What I would do personally (i.e. not backed by some official rules):

If the above still does not yield anything, contact ED Support. After that, you might be left only with either a) your mom or b) the bartender of your fav. pub.
