There seems to be the same bug with Mi24 that Mi8 had back in the day: 

 

 

This is literally the same issue, and it occurs if one doesn´t hold the trigger through the whole sequence, instead rapid fires. I´ve seen this now many times with both S-5s and S-8s. I can provide a track if needed.
