missing track file AGM-84E missile orbiting around launch point

I was trying AGM-84E and find out approximately 50% of missiles are failing and starts a orbit around launch point and explode mid-air.I tried both PP/TOO mode at different altitudes with different fly-out profile.I also test launcing directly to the target or 15 to 60 degree offset.It doesnt matter and couldnt find out what causes the problem.

EDIT:

-I have been testing for hours and as far as i found it is about offset heading (Although i wrote that it doesnt matter if I launch directly to the target or not).

-I did not have any problem with +/- 20 offset from target but more you go it is more likely to do orbit because missile trying to intercept attack heading and overshoot its course and once it overshoot it cant re-correct itself and continue to orbit until it explode
