Running the starter

IIRC there used to be a key command for the crew to run the inertial starter. Now I can't for the life of me find it. It's only available via the radio menu. Has this been removed?

Yes, the keybinding command was removed, about two years ago.

By the way, we can't see the starter crank spinning or the ground crew cranking it.

... only in a supercarrier you can see ground crew servicing or doing help to an airplane. I think?
