Finding out who/how/why did wrong to get to this disaster is a debate about the accident a think you will agree. BTW Boberro clear your mail box, it`s as full as a good child`s socks are full of toys on Christmas. :) Few months ago I would be all over the subject, drawing figures and what not but now not that much. The first reason is the consequences I mentioned above. It was a human error. 3 main factors in death triangle and there you go with tragedy. In this case: weather, stress and broken procedures... Oh and the audio record is indeed horrible... Putting the naming conventions aside there were enough of them (certainly not 3) to make 10 of such aircraft crash. 100 people onboard, LESS than 300 m visibility, LESS than 50 m cloud base... COWABUNGAA! I used to say that you can't blame the crew directly because of two things: - they were supposedly under pressure - they are a product of certain sick system, forcing them to act in a certain way Now I'm not so sure. When your child is not dying you tell such life-gambling superiors to go *themselves! If you don't, the superiors are not the ones to blame. Than as a citizen you could come to only one realization - if that tragedy didn't make someone re-think everything on the high levels NOTHING EVER will shake them of their lethargy. May I remind you that we already had a "prelude" crash to that of Smolensk - CASA C-295M crash on 23 January 2008 http://en.wikipedia.org/wiki/Mirosławiec_air_accident Over 20 Air Force officers died, including some high-rank ones. What has changed after that? Oh, it changed. For worse (general situation in Polish Air Force). Smolensk as well as many other incidents showed that no lessons were learned. GEN. Because it's damn tail-heavy, that's why! :D Beautiful scenery though :) As for the crash being an opportunity to shift Polish air safety culture from Eastern to Western - after few brief talks with my colegues from Polish Air Force Academy it seems the disaster had some impact on, at least, curriculum changes. For one students' initial training is according to civil aviation regulations. Two - it is said that there is easier now to get kicked out of school due to serious mistakes during training flights. Not too much info, I know. Plus it is too soon to see if the small changes so far make any actual, long-term difference.
