Predesignating Marked Pages on Kneeboard

First of all, it might be noteworthy, that you can save kneeboard pages under the saved games folder of the respective aircraft kneeboard so they will always come up for that type of aircraft. 
You can also save them under the terrain, so they will always come up if you fly in that terrain (this is in the main folder though, I believe), no matter which aircraft.

Now I get into the realm of speculation: I believe, you can open a .miz file like a zip-file and there should be a kneeboard folder there as well. 
 

I know, that this is not quite the answer to your question, but hopefully it is still helpful.

No native way to do it.  You'll have to use some sort of scripting (i.e. AutoHotKey) to press the appropriate sequences of buttons to set them up as as marked tabs

It can be done for Single Player missions, if the mission is edited by yourself. You need to use the "x:perform clickable action", and "x:set command" triggers to basically page forward the KB and place the tabs at the appropiate points. Fortunately, it can be done without having the KB displayed, so that any flickering is invisible, it looks like this:

 

 

The triggers look like this:

The only downside, is that these trigger need to be updated if the amount of pages on each section changes.
