The cause is one engine inlet intensive icing. The pilot shouldn't attempt take-off before having indication that the engine is fully deiced. See, that's why I hate the Internet, the first days after the incident all Russian news agencies said iy was the pilot :( Now I don't care who that person was, it doesn't make it any difference, but the misinformation/conflicting reports are lame.
