I've experienced an issue where flying on the belly tank - selector set to external tank and source selector set to belly - that the needle stayed pegged at full fuel for a while with no decrease in fuel levels until the engine cut out, at which point I switched to internal fuel and the engine came to life. Shouldnt the fuel guage for the external tanks have read zero?

No fuel gauge for external tanks.

That is how fuel tank selectors work in P-47 and similar in P-51 as well.

Main selector have 3 position Main tank, Aux tank, External tanks. When set to external tanks you can use 2nd fuel selector bell left and right wing tank.

System is setup that way that from only one tank engine can be fed at a time.

For example external tank in Dora is pressurized by engine's supercharger and fuel is forced out in to rear tank, so in case of Dora, when pilot see that rear tank fuel level goes down this mean that external tank is empty.

In p-47 or p-51 fuel pressure drop is only indication before engine quits.
