After starting DCS the throttle/collective is set at 50% despite 'Synchronize Cockpit Controls with HOTAS' being checked in the options.

It seems only to happen the first mission after starting DCS; subsequent missions during the same session obey the Synchro option.

Watch the collective indicator top left after spawning into mission. After switching to a jet the same thing happens - it starts to accelerate down the runway.

That's because you haven't set the axis as a slider in the axis tuning of the collective axis in the Control Settings.
Throttles, brakes, collectives, volume controls etc. These are all sliders. Only sticks, ministicks, rudders etc. should not be sliders.

Even set to slider it still does it. Watch...
 

Sometimes the forums does strange things with quotes...

Watched the video, can't think of anything else but axis conflicts then. I do have it inverted too, as I'm using a collective.

This is known bug. Probably something with TM device initialization in DCS. Has nothing to do with slider setting.

Only workaround is to do one input signal on the device before the mission - either move any axis or push any button.
