It is only “OH58D”.

There is a good instruction file included.

Funny fact: When I started making these checkists In 2017 I didn’t have VR (only got it 6 months ago) and I never understood the Vr guys saying the checklist were good for Vr until I tried it. LOL

The design fits VR nicely by pure chance. Anyway, I will keep using the same style.
