When I first saw this, I thought it might have to do with air temperature or wind, but since then I've seen a few failed engine start attempts at some of the most standard atmospheric conditions. Looks kind of random to me.

indeed it does appear random.  Happened to me last night on a MP mission.  Second engine just wouldn't start up.  Ng% never goes above RED and the engine spools down back to 0%.  I spawned into a new helo and performed exact same start-up sequence I always do ... started up, no problem.

This was the first time experiencing this "failed to start" issue after weeks of heavy play.  Go figure.

You’re supposed to wait for the other engine to stabilize at idle speed, somewhere around 67% NG or so before starting the second engine. You’re doing the start procedure incorrectly. The aircraft is not capable of supplying enough air to start both engines at the same time.

we should ask the crew chief to stick a leaf blower in the #2 engine intake, maybe then it will have enough air.

The engine inlet isn’t the air source.

I only ever saw this problem if I left the collective up (blades tilted up and air resistance high) after bore sighting, the result was always trouble starting engine 2.

Apart from that, engine 1 & 2 always starts for me if the proper conditions are correct.... perhaps moving the collective works in my advantage?

Same for SP or MP.
