Just curious, whereas the ladder was designed to be hooked, does crews actually hook it more, or just place it on the ground more?

Its reported. The lower position a sort of Standby position. Over the canopy rail when someone actually wants to get in.

Hello guys,

Many apologies if it was already mentioned, but I didn't manage to find a bug report for this. 

During cold and dark starts, and at any time when the ladder is requested, it is installed in the wrong position. Resting on the ground, and against the fuselage. The model of the ladder which is in the DCS Version of the MiG-29 was designed to hook around the canopy, rest against the fuselage, and the bottom suspended in mid air.

No any issue here. You have several options for attaching the ladder. This is already discussed in the post above.
