How do we know when the tank is killed? Do they always blow up/smoke?

Reading the great book "warthog" by William L Smallwood, and the author talks about pilots diving on Tanks, firing into the top/rear, at 8,000-10,000' with the Gau-8 and "often they were not able to start the tank burning-or at least cause a fire they could see. Most of the tanks that were hit burned inside, but they did not find that out until after the war."

Yes, in the game any destroyed vehicle will always explode. If they're "only a little bit destroyed" they'll burn for a bit and then explode. If they're "very thoroughly destroyed" (e.g. you dropped a bomb right on top of it) then it explodes immediately.

I took out a T-72 in beta 4 with a short burst the other day. The key is to get close. I didn't fire until I was at half a mile, perhaps even less. Very nearly flew right into the damned thing! I actually didn't think it would kill it since I don't have normally have much success taking out tanks with the gun and I was quite low on ammo, about 120 rounds remaining I think. So I went off and took out a BMP first, and then came back to the tank with ~ 80 rounds left. Was quite surprised when I smoked it; but it clearly doesn't take all that much, so long as you actually hit it.

Did they adjust gun effectiveness in Beta 5 as well?

No - already tweaked since Beta 4. Takes approx 110 rounds to cook a T80 if done within recommended parameters.

Where did you download it to? Have you checked your downloads folder? You will need to copy and paste the track file into the tracks folder from whatever folder you saved it to when you downloaded it.
