It can be out of sync for two reasons: faulty electrical connections in cables, software error in the BTsVM. As for 90's manuals stating it's out of sync - it's indeed an ass covering document to cover MOD not functioning properly. Overall this delta H + range radar cone steering is really relevant in maximum altitude separation and closer ranges, aka GCI or buddy aided ambushes. In "free hunt" it's more likely you'll look for max range search where cone will be wide enough.
