Just to make sure. You haven't bound it to an axis, and have selected sync HOTAS at mission start in the general settings?

Actually, I believe you're right. I think it's always been off. At least for the QS AAR missions. I never use it, so I've never noticed.

1. Yes and yes. 
2. My older bfm videos can prove they were on by default, especially starting in mid-air. Same as the F/A-18C.

Well, I can map the HMD intensity to an axis to make it on by default.

If you want it to be on, bind it to a flip switch on your hotas that you dont need. Set DCS to sync with hotas at simulation start. It will dial up the HMD as soon as youre in the jet.
