Lately either I am just really unlucky or a bug.  No matter what I get hit with, pilot is dead and can't eject.  The aircraft can still look undamaged but dead pilot.  My AI counter parts however seem to always be able to eject even if I hit them with multiple missiles.

Anyone else seeing this?

Unfortunately, without a track there's really no way to analyse what you are saying.

Pilot survivability in the F-16 has been discussed here many times and all we can say is that the canopy design on the F-16 does indeed expose the pilot a bit more than other aircraft and it is very likely for bullets or just shrapnel hit the pilot. I'm not saying this logic is perfect in DCS, if there's anything to improve in this regard we may refine it in future updates.

Main issue seems to be where the missiles in DCS impact the jet vs. where they impact in real life, as opposed to an issue with the damage model itself. There have been a number of real-world ejections from Vipers hit by SA-2, -3, and -6; one instance in which the airframe was cut in half

Yep, the DCS F-16 (if not all the planes) point of impact is exactly where the pilot, or even pilot's head is, if I remember correctly from what I've read a while ago. So every single weapon toward you, will going straight to pilot's head. So why not being dead in almost all the cases?
