It's called ground stabilised and real Apache never had it.
You have to establish manual track as best as possible, lase, engage LMC and correct as needed.
I engaged a column that was moving in the open from right to left in SP. If I track manually (with slight wobble on TADS) every single missile would hit behind the column 100-150 meters. If I track with IAT, every missile hits its designated target
Check settings in DCS and see that you get to 100% in game at full throw. This seems like calibration issue, where stick sends full input when it still has some throw.
I tried it too.
Out of 8 RF missiles, only one hit different target in a group, when I did a 360 descent to mask. All others, where I masked with nose on target, hit designated targets.
If you used same keyboard combination for both positions, go to CPG tab and check what happens when you press CTRL+V. If nothing happens, click on keyboard column and press restore to defaults.
What's that got to do with AI menu?
Can you record another track that shows engine instruments? To me, it sounds like rotor was spooled down?
Also, create your own mission and test this, training missions are behaving weird sometimes.

I just got into hover at 5000 ft MSL/10°C FAT with 1880 lbs of fuel, 1 rocket pod/2 hellfires on each side.
Are you trying to get into hover overweight for the conditions?
Look, I am not saying that there are no problems with the way George is flying, I am just saying that there are ways to circumvent it.
I did get into VRS if I comand him to stop into hover if he is climbing or descending to maintain radalt. I try to fly alongside mountain side when I want to get into hover. That way it is easier for George to maintain radalt and he manages to get into hover.
Edited to add: look at your radalt before you give him comand to slow down. Give him higher radalt that currently is and he will climb into it while losing speed. That way he will not be too low on collective to let helicopter go into VRS.
You need to tell him where to look, what target to lock onto and (if not in free fire mode) give him permission to engage. Also, you need to make sure that you are positioned in such way that there would be no inhibit messages in HAD

Interestingly enough, he is quite able to get me into hover without killing us, so maybe try to change how you do things until George gets fixed?
